Approval of a New Protocol

There are three possible types of review and three forms for new protocols.

What form should I use?

If your research is eligible for exemption, use the Request for Screening for Exemption.

If your research needs expedited or full review, use the Request for Protocol Approval. The form allows you to indicate which type of review you think is appropriate.


If your research involves the analysis of identifiable data collected by someone else, use the Request for Approval for Secondary Analysis of Existing Data for all three types of review: exempt, expedited, and full.
