This form should be used to establish a limitation of F&A/indirect costs when the sponsor is limiting such recovery or when the PI wishes to request a waiver of Duke's F&A/indirect cost rates.

The attached sample questionnaire may be used by departments to gather basic information from PIs in order to create an SPS entry for a proposal. This sample questionnaire was develeped by the Department of Chemistry at Duke University.
